Artifact Description:
What is the tool that you used to create your form?I created this from using Google. It was an easy site to use, especially for me who had never before used Google as a way to make a sharable document. I also like how Google saves your work as you go to ensure no progress is lost.
What process did you go through to create the form?
In order to create my student survey, I first thought of questions that would force students to share their at home study habits, evaluate their time management, and acknowledge areas of possible improvement. After creating questions for each category, I then chose the format for each question. For example, some questions are better suited for multiple choice or check-box options than others. Following designing each question's format, I proofread and took the survey myself to check for any errors.
How does it fit into the component you identified?
The google form connects to Component 1b, demonstrating knowledge of students, because by utilizing technology, I was able to create a document that would allow me to learn more about students. I used google as a resource to better my own understanding of students. Furthermore, as a future teacher, the form would allow me improve my classroom dynamics because I would be able to have a better understanding of my students' study habits. With this information, I could help students improve their test scores by encouraging them through the study process. Lastly, by using technology I can get to know about my students' personal lives such as their extracurricular activities, likes, and dislikes.

How can you use a google form in your future classroom?
In the future, I can use a google form in my classroom in a myriad of ways. For example, I can utilize a form as a "getting to know you" activity for my students. By using a google form instead of a hand-written worksheet, I can more efficiently read each response allowing me to learn more about my students. I can also assign a form as a homework assignment that includes questions that are relevant to the material learned in class. This will reinforce subject content and allow for efficiency in submitting work.
How does it relate to growth mindset?
Using a google form relates to a growth mindset for both the students and the teacher. On the student end, a google form provides children with the opportunity to reinforce information by completing homework or other activities. After completing the forms, students will have a better understanding of their ability to grow their minds. Conversely, teachers can ensure they never stop learning by constantly introducing themselves to new concepts. Utilizing new technology such as a google form allows teachers to constantly be pursuing innovative ideas that will surely grow their thoughts.
What did you enjoy about creating the artifact?
I really enjoyed using a google forum to create my student study habit survey. The site is extremely user-friendly and encourages the collaboration of students and teachers. Furthermore, I appreciate the ability to share the document so that all students (or parents) have access to the content. The site's openness and accessibility allow for easy communication among teachers, students, and parents. I will definitely use this resource again!
